1. Health Certificate via Foreigner Physical Examination
Make an appointment in advance via the official WeChat account of Hunan International Travel Health Care Center. Physical examinations are available from 8:30 to 11:30 a.m. on the scheduled date. Applicants must be on an empty stomach. Items include infectious disease screening, internal and surgical examinations, B-ultrasound, electrocardiogram and laboratory tests.
2. Vaccination
Available upon application. Service hours: 8:30-11:30 a.m. and 14:00-16:00 p.m. on working days. Vaccination can be arranged on the same day of physical examination.
3. Health Management, Consultation and Publicity Services
We provide Chinese-English bilingual publicity materials on infectious disease prevention and control, online health education and popular science services, as well as free offline English health consultation.
List of Application Materials
1. Authorization Letter for Disease Surveillance Report(original edition)
Those who have obtained physical examination reports or verification certificates in advance are exempted from submitting this document, and only need to submit the original and copies of such reports or certificates directly.
2. Supporting documents for alteration application(original edition/electronic edition of the original/copy)
(1) For change of passport (international travel document) number: submit the new passport number together with copies of its information page and visa page.
(2) For internal job transfer within the same employer, including promotion from professional posts to administrative posts: submit an alteration application letter and relevant supporting documents, subject to other provisions of national laws and regulations.
(3) For transfer to a new occupation or job position: cancel the existing work permit and apply for a new Foreigner Work Permit in China.
(4) All non-Chinese documents shall be accompanied by Chinese translations affixed with the official seal of the employing entity. All original paper documents and their Chinese translations shall be electronically uploaded to the official service system.
3. Evidences concerning termination of employment relationship, expiry of labor contracts or other documents related to cancellation reasons(original edition/electronic edition of the original/copy)
(1) Termination of employment relationship and labor contracts shall be signed by both parties.
(2) If the applicant resigns voluntarily or cannot be contacted by the employer, the employer shall additionally submit a written explanation on the cancellation circumstances.
4. Non-criminal Record Certificate(original edition/electronic edition of the original/copy)
It shall be issued by police, security or judicial authorities of the applicant’s country of nationality or habitual residence, and authenticated by China’s overseas diplomatic missions or foreign diplomatic missions in China.
Non-criminal records issued in Hong Kong, Macao and Taiwan regions shall be notarized by local notary institutions.
Habitual residence refers to the country or region where the applicant has resided consecutively for more than one year before leaving his home country, excluding mainland China.
The certificate shall be issued within 6 months.
A commitment-based system is applicable to Category A high-end foreign talents.
Pure self-declared sworn non-criminal statements are not accepted.
Non-sworn non-criminal records issued by foreign diplomatic missions in China can be directly accepted without further authentication.
All non-Chinese certificates must be attached with officially sealed Chinese translations.
5. Business license, organization code certificate and other registration documents of the employer(original edition)
Exempted for registered employers with non-first-time applications.
6. Original health check records issued by medical institutions in the home country/region(original edition)
The original records including physical examination and laboratory test reports shall bear the official seal of the medical institution and the signature of the attending physician.
The records are valid for 6 months. Only documents in Chinese or English are acceptable; those in other languages will be rejected.
Issuing overseas medical institutions shall be public hospitals designated by Chinese consulates abroad. Health certificates issued by private hospitals must be notarized by local notary authorities.
7. Application Form for Cancellation of Foreigner Work Permit in China(original edition/electronic edition of the original/copy)
Complete and print the form online, affix the official seal of the employing entity, then upload it to the system.
